League Physicals!
Saturday, June 20th at No Limits Mixed Marital Arts (1962 Barranca Parkway, Irvine 92606) from 9:00 a.m. to 3:00 p.m.
Please bring the following:
1) A copy of their birth certificate; they MUST bring an original copy of their birth certificate; this is a new rule that they must abide by!!!
2) Copy of utility bill - For families w/ more than one player/cheerleader, please bring a copy of their utility bill for each child
3) A small color passport size color photo
4) Full year report card for the 2008-2009 school year
